Session request form

Existing clients with a PDA or cell phone equipped to read HTML should use the simpler Appointment request form, which is specially designed for mobile devices.

Lady Lydia asks both new and existing clients to submit their session requests using this form whenever possible. If you have any difficulty using the form, please email equivalent information to Lady Lydia and explain the difficulty.

New clients should first study the other pages of this web site, especially the Possibilities page[www.seattledungeon.com/LadyLydia/activities.php]. Then, please take the time to complete this form setting out your experience and your wishes. And do so truthfully because Lady Lydia will use your answers to plan your session.

It isn’t always necessary to complete every section of the form. The first section, About you, should always be completed but if your only interest is a corporal punishment session, it isn’t necessary to complete the sections for bondage, sensation play, etc. However, do make sure to provide all information relevant to the session you are requesting.

Existing clients need not detail their interests and experience unless requesting a session not previously discussed with Lady Lydia.

Remember that Lady Lydia usually requires 36 hours notice for a session reservation and that proper forethought and planning will improve the quality of your session.

The information you submit to Lady Lydia will be kept completely private. It will never be released to third parties.
